Providers: pharmacists
Interventions 1. Initial counselling session from hospital pharmacist, up to 16 further weekly meetings after discharge, nicotine patch dispensed weekly at visits.
 2. Initial counselling from hospital pharmacist, further sessions with a community pharmacist, same availability of patch
 3. Minimal intervention, brief advice only
