adefined as the presence of one or more of: history of cardiac disease, hypertension, diabetes, venous thrombosis, pulmonary embolism, Graves’ disease, asthma, homozygous sickle cell anemia, thrombocytopenia, coagulation disorder, a rare or systemic disease, nephropathy, HIV infection, psychiatric disease
