Time interactions were added to each model separately. All models were adjusted for age, time, education level, time-varying per capita GDP and time-varying per capita GDP×education interaction, except for the baseline per capita GDP model, which used baseline per capita GDP instead of time-varying per capita GDP.

P for age×time interaction=0·07 and 0·10 for men and women, respectively. P for linear trend across age quintiles=0·15 and 0·01 for men and women, respectively. Age was categorized for this model only.

P for education×time interaction=0·49 and 0·99 for men and women, respectively.

§

P for baseline per capita GDP×time interaction=0·66 and 0·51 for men and women, respectively.

||

P for change in per capita GDP×time interaction=0·19 and 0·18 for men and women, respectively.
