Decision Chart for Administration of the Neuropsychological Assessment Battery

IF MoCA change is:	THEN ADCS-CFSI Score to trigger the neuropsychological assessment must be:
1 point	4.0 or greater
2 points	3.5 or greater
3 points	3.0 or greater
4 points or greater	Administer neuropsychological battery regardless of CFSI score
-or- Year 3 MoCA score 25 or less after education points added
Self-report of MCI or dementia diagnosis
-or- Randomly selected normal subsample
