Model includes all 9 recommendations together in a multivariable Cox model and additionally adjusts for age, tumor stage, ER status, chemotherapy, Herceptin, radiation, comorbidities, smoking status, income, and education level. Mortality cohort (n= 1,736); Recurrence cohort (n=1,705). No collinearity issues were identified.
