Checklist for primary care practitioners (some could be done by trained receptionist/administration) Checklist for patients
• Do I need to see the patient – if so, is a video consultation possible?
• Am I aware of this patient’s needs?
• Can I access their medical history?
• Do I know the patient’s goals?
• What is the patient’s physical, smoking and mental health status?
• Should I be expecting the patient to give me any test results, medical diary or completed questionnaire?
• Does the patient have access to a phone, smartphone, tablet or computer?
• Does the patient have access to respiratory function testing equipment?
• Can the patient use the technology correctly?
• Is the patient’s family/home condition supportive?
• Have I completed any tests, diary or questionnaire my primary care practitioner has sent me? (I may prefer to complete these with my primary care practitioner during the consultation)
• Have I prepared a list of questions for my primary care practitioner?
• Which symptoms are bothering me most at the moment?
• Do I have my medications to hand, including my inhaler(s)?
• Am I in a quiet private space?
• Do I have a pen and paper to hand to make notes?
• Do I have my glasses with me (if I need them)?
