| 0 | Intact normal mucosa, without evidence of hyperemia |
| 1 | Intact mucosa, with areas of hyperemia |
| 2 | Small, single or multifocal superficial lesions |
| 3 | Large single deep or multifocal superficial lesions |
| 4 | Extensive lesions with areas of evident deep ulceration |
