Physical Examination Positive Tinel sign over the carpal tunnel
Positive Phalen test
No sensory changes over PCBm distribution
Negative Gainor compression test
Treatment Symptomatic relief with CSI into the carpal tunnel
EMG Confirm no intrinsic nerve pathology
Document lack of nerve injury
Rule out “Mimickers” TOS
Cervical pathology
Supracondylar process syndrome
Pronator syndrome
Lacertus syndrome
AIN syndrome

CSI, corticosteroid injection; PCBm, palmar cutaneous branch of the median nerve.
