Technical Considerations for the Adipofascial Flap

Technique for Adipofascial Flap
1.Perform after ulnar nerve decompression and anterior subcutaneous transposition
2.Mobilize the adipose flap of approximately 12 cm x 4 cm from the medial elbow
3.Leave a thin layer of adipose tissue on the dermis to maintain adequate vascularity of the skin
4.Take care to avoid injury to the adjacent branches of the medial antebrachial cutaneous nerve
5.Transpose the flap, so it encircles the nerve in a posterior-to-anterior direction
6.Suture the flap to itself with absorbable sutures
7.Range elbow to ensure free nerve gliding and no kinks
