Technical Considerations for the Adipofascial Flap

Technique for Adipofascial Flap
  • 1.

    Perform after ulnar nerve decompression and anterior subcutaneous transposition

  • 2.

    Mobilize the adipose flap of approximately 12 cm x 4 cm from the medial elbow

  • 3.

    Leave a thin layer of adipose tissue on the dermis to maintain adequate vascularity of the skin

  • 4.

    Take care to avoid injury to the adjacent branches of the medial antebrachial cutaneous nerve

  • 5.

    Transpose the flap, so it encircles the nerve in a posterior-to-anterior direction

  • 6.

    Suture the flap to itself with absorbable sutures

  • 7.

    Range elbow to ensure free nerve gliding and no kinks
