aResource utilization and costs for drug acquisition, drug administration, inpatient and outpatient visits, and ADR management were derived from publicly available reference sources: the Micromedex database for drug cost information and product labels for dosing and administration information.19 Given the differences in dosing and administration frequency, we applied drug costs on a weekly basis as determined by the annual cost per course divided by 52 weeks.
