aResource utilization and costs for drug acquisition, drug administration, inpatient and outpatient visits, and ADR management were derived from publicly available reference sources: the Micromedex database for drug cost information and product labels for dosing and administration information.19 Given the differences in dosing and administration frequency, we applied drug costs on a weekly basis as determined by the annual cost per course divided by 52 weeks.

bVedolizumab 300 mg IV on day 1 and weeks 2 and 6 (induction) and every 8 weeks thereafter (maintenance).

cAdalimumab 160 mg SC on day 1, adalimumab 80 mg SC at week 2, adalimumab 40 mg SC at weeks 4 and 6 (induction), then adalimumab 40 mg SC every 2 weeks thereafter (maintenance).

dCosts of ADRs and other direct medical costs were based on CPT and DRG codes.

eAll patients received initial or subsequent treatment for the entire model horizon; thus, costs of routine monitoring are equal for both treatment arms.

ADR = adverse drug reaction; CPT = Current Procedural Terminology; DRG = diagnosis-related group; IV = intravenous; SC = subcutaneous; USD = US dollars.
