Excision of the sinuses. The sinuses are excised, leaving a rim of approximately 5 to 8 mm of aortic wall adjacent to the cusp insertion. The coronary orifices are mobilized. We usually excise the noncoronary sinus first. Most of the respective sinus wall is left on the coronary buttons.
