Interventions to improve the impact of cognitive impairment and frailty in HF
  • Screening for cognitive decline using available tests (the Mini-Mental Examination)

  • Evaluation impact on self-management skills

  • Stimulating exercises that result in improvement in memory (activities involving learning strategies remembering verbal and visual information, tasks such as categorical naming, “find the differences” and encoding used to increase attention and processing speed exercises)

  • Use of external memory aids such as notes, calendars, or other resources; strategies to improve episodic memory (list recall, face name association and text memory)

  • Assertiveness training

  • Creation of an education and training plan
