Examples of what might be considered insanitary conditions for compounding drugs in pharmacies or physician offices

  • Performing manipulations outside of the designated area

  • Microbial contamination of designated areas

  • Loose or unsealed ceiling tiles

  • Infrequent or no use of sporicides in the clean rooms

  • Improper donning of lab apparel

  • Contamination of lab apparel

  • Presence of vermin, insects, rodents observed in production areas, or areas adjacent to it
