Twenty-four-year-old female transplanted for, histologically proven, lymphocytic interstitial pneumonia who developed restrictive chronic allograft rejection 20 months after the transplant. In A an axial image of the high-resolution computed tomography performed before the bilateral lung transplant well demonstrating bilateral severe interstitial thickening, bronchiectasis, and fibrotic ground-glass as well as cysts in the left lower lobe. In B an axial high-resolution computed tomography image demonstrating peri-hilar consolidations on the right side as well as band atelectasis and ground-glass opacities in the lower lobes; in C an axial image of the expiratory phase showing air trapping in the lower lobes.
