Where the nature of the target population, or the funding mechanism, does not allow for equal numbers some attempt should still be made to recruit a substantial number in each sex and gender group. Where differences in outcomes are unimportant, or space limitations preclude detail when publishing, this should be commented upon, and stratified results included in a web supplement to inform potential future meta-analyses
