Major trauma, surgery, head injury in last 3 weeks	Traumatic resuscitation
Bleeding diathesis/active bleeding	Use of ECMO
	Advanced liver disease
	Infective endocarditis
	Active peptic ulcer
	Refractory hypertension (systolic BP > 180)

ECMO indicates extracorporeal membrane oxygenation.
