• Advanced age
• Male sex
• Family history (short telomeres)
• Clubbing of the fingers
• Need for oxygen therapy
• > 20% extent of fibrosis on HRCT
• Extensive traction bronchiectasis on HRCT
• FVC of < 50-65% predicted
• DLCO of < 50% predicted
• An SpO2 of < 85% during exercise
• No identification or avoidance of antigens in CHP
• Pulmonary hypertension
