A 52-year-old male with increasing pain and immobilization 6 years after THA; progressive stress shielding distally around the stem and loosening zone according to all Gruen zones; caused by aseptic loosening stem revision from a tapered rectangular stem to an short stem; in axial view, the relatively broad shoulder region compared to the explanted stem allows sufficient bone stock and provides superior rotational stability; from the first to last follow-up even new re-ossifications of the persisting loosening zones around the mid-stem.
