Colistin
Loading dose: 9 MU irrespective of baseline creatinine clearance
Maintenance dose is given 12 h after the loading dose based on creatinine clearance (mL/min) as follows

<50 and >90 (for patients weighing >90, adjusted body weight* should be used to calculate the dose)	Loading dose: 3 mg/kg/dose
	Maintenance dose: 1.5 mg/kg every 12 h
