aModerate risk factors include a BMI between 30 and 39 kg/m2, asthma, chronic kidney disease, diabetes mellitus, hypertension, pregnancy, and smoking (past or current).
bHigh risk factors include a BMI of >40 kg/m2, metabolic syndrome, cirrhosis, cardiovascular disease, chronic obstructive pulmonary disease, end-stage renal disease, immunocompromised status, frailty, and living in a health care facility.
