The study team will formulate and finalize recruitment plans in year (Y) 1. The intervention will occur in 3 waves per year over Y2 to Y4. Participant focus groups (FGs) will be conducted at the end of Y2 to Y4. Cardiac rehabilitation (CR) staff and stakeholder FGs will occur at the beginning of Y5.
