Patient diagnosed with congestive heart failure (mcql60b) or heart attack (mcql60e).
Blood pressure	Adults 18–85 with at least one face-to-face visit and an active diagnosis of essential hypertension any time prior to the first day of month 7 of the measurement period; excluding (i) pregnant women and (ii) patients with end stage renal disease, dialysis, or renal transplant prior or during the measurement period.	Patient diagnosed with hypertension (bpq020) who are not currently pregnant (rhdl43), have a failing kidney (kiq022) or received dialysis in the past 12 months (kiq025).
