Tell me about the chronic conditions that you have. When were you diagnosed with those? What was the process of learning to manage your condition(s) like?  • What kind of resources or supports did you have along the way?  • What things made it more difficult for you to manage your conditions?  • How did you experience your interactions with healthcare providers who tried to advise you on how to manage your conditions? What is self-management like for you now? What makes it that way (probe for positive/negative influences)? Where would you like to see your self-management in the future?
