cRelationship to the insured refers to the participant’s status as the primary beneficiary of the insurance policy. “Self” indicates the participant holds the policy in their own name. “Spouse” denotes the participant is covered under a policy held by their married partner. “Dependent” means the participant is covered under a policy due to their status as a dependent, typically a family member without independent coverage.
