An attempt at conservative treatment with a thumb extension splint was unsuccessful and led to an indication for surgical treatment. (a,b) Preoperative photographs. (c,d) A four-flap Z-plasty and release of the intrinsic tenar muscles was performed to widen the first web space, and an MCP joint stabilization (double breasting of the capsule and K-wire fixation) and a tendon transfer of the extensor indicis were also performed.
