The first site of persistent immune-mediated inflammation is most likely the microvessel wall, causing alterations involving not only the endothelium, but also all the vessel layers, with subsequent myofibroblast activation, excessive extracellular matrix (ECM) deposition, and unrestrained tissue fibrosis.
