* Perinatal outcomes are defined as outcomes occurring within the first 24 hours of life.
** Severe neurodevelopmental disability was defined by a previous Cochrane review 39 ; “any one or a combination of the following: non‐ambulant cerebral palsy, developmental delay (developmental quotient less than 70), auditory and visual impairment assessed at 12 months of age or more. Development should have been assessed by means of a previously validated tool, such as Bayley Scales of Infant Development (Psychomotor Developmental Index and Mental Developmental Index” 40 .
