• Assessing the effect of the COVID-19 pandemic on breast cancer incidence, stage at diagnosis and treatment must consider population-level historical trends
• Incidence of early-stage breast cancer was lower in the first wave of the pandemic but recovered in the second half of 2020 in Belgium
• At the national level, no shift to more advanced stage at diagnosis was observed during the second half of 2020 in Belgium
• Treatment adaptations, such as increased use of neoadjuvant hormonal therapy to delay surgery, were appropriately applied
