Image A shows a severely fragmented fracture of the trochanteric mass with nearly total destruction, which poses a very difficult situation for orthopedic treatment. Image B shows the utilization of a Gamma nail to stabilize the fracture. The selection highlights the appropriateness of the Gamma nail for ensuring secure internal fixation in situations where the remaining bone structure is severely compromised. The use of a dynamic hip screw (DHS) system at this point would have increased the risk of further compromising the strength of the remaining bone, emphasizing the importance of careful decision-making when dealing with severe fractures.
