For visit level analysis (appropriate or acceptable treatment), both practice and participant were included as random intercepts. For other participant level analysis, practice was included as random intercepts. The region, baseline appropriate antihypertensive treatment rate (≥median or <median), and calendar time were included as fixed effects in all models.
