Arthroscopic view from the posteromedial portal of the posterior compartment of a left knee. A PCL tibial guide pin was placed close to the native PMB footprint (A) and a tibial tunnel of 6 mm was drilled under posteromedial visualization (B). A 6-mm in-out femoral tunnel was subsequently drilled next to the attachment site of the PMB, which is outlined on the wall of the intercondylar notch and distal to the medial arch point (C, arthroscopic view from the high anterolateral portal). The graft was routed from the tibia to the femur and first fixed on the femoral side with a bioabsorbable interference screw (D).
