A 22-year-old male soccer player presents with severe pain during the match and a free gastrocnemius aponeurosis injury at 36 hours was diagnosed. Long axis US image (A) shows an altered echostructure with disruption (arrowheads). The distal fragment of the disruption shows a fusiform and thick shape. The muscle echostructure of the medial head of the gastrocnemius is minimally altered (arrow). Axial (B) and sagittal fat-suppressed TSE intermediate-weighted images (C) show a clear isolated gap in the free aponeurosis (arrowheads). There is no remarkable hematoma.
