  • Has this child ever had a special education or early intervention plan? Children receiving these services often have an Individualized Family Service Plan or Individualized Education Plan.

  • Has this child ever received special services to meet theirb developmental needs such as speech, occupational, or behavioral therapy?

  • Is this child currently receiving these special services?

  • During the past 12 months, has this child received any treatment or counseling from a mental health professional? Mental health professionals include psychiatrists, psychologists, psychiatric nurses, and clinical social workers.

  • At any time during the past 12 months, did this child receive behavioral treatment for ADD or ADHD, such as training or an intervention that you or this child received to help with theirb behavior?
