Study flow diagram. Flow diagram outlining patient selection criteria and approach strategies for CAS procedures performed at our institution from October 2018 to September 2024. Patients were excluded if CAS was performed as a retreatment following prior CAS or carotid endarterectomy (CEA) or if CAS was performed concurrently with mechanical thrombectomy (MT) for acute ischemic stroke (AIS). The cohort was divided into two periods based on the preferred access strategy: the TFA-first period (October 2018 to March 2023) and the TRA-first period (April 2023 to September 2024), reflecting an institutional shift from TFA to TRA as the primary strategy for CAS starting in April 2023.
