Management of non-obstructive azoospermia: advances, challenges, and expert recommendations

This special issue of the Asian Journal of Andrology is fully dedicated to the thematic area of non-obstructive azoospermia (NOA), one of the most complex and challenging conditions in the realm of andrology, urology, and reproductive medicine. As a distinct form of male infertility characterized by severely impaired or absent spermatogenesis, NOA has long captured the attention of researchers and clinicians due to its diagnostic intricacies, limited treatment options, and often unpredictable outcomes. Despite significant advances in microsurgical techniques, hormonal therapies, and genetic testing, the management of NOA continues to raise critical questions and debate. This issue brings together expert perspectives to examine the latest developments, ongoing controversies, and evolving recommendations aimed at optimizing care for men with this difficult condition.

The prevalence of male infertility has risen over the last several decades and continues to increase.1 Multiple factors account for the deterioration of semen parameters, including environmental, genetic, and lifestyle influences.1 Although every infertile couple has challenges to overcome, NOA is arguably one of the most challenging and complex scenarios encountered in reproductive medicine.1,2,3,4

Approximately 1% of men in the general population, and 10%–15% of men presenting for fertility evaluations, will be diagnosed with azoospermia.5 NOA is a condition with no man anticipates, and despite significant improvements in diagnostic and treatment options, there is still a high proportion of couples who fail to achieve pregnancy and live birth. NOA requires the highest levels of clinical care for the male and female partner, supported by advanced laboratory technology, to offer an opportunity to these couples to conceive with their gametes.

The management of NOA requires a multidisciplinary effort among reproductive urologists, reproductive endocrinologists/gynecologists, and embryologists. This journey can take months or even years as couples navigate the complex landscape of medical and surgical interventions. Treatment approaches vary based on the underlying etiology. These may include the correction of reversible factors, medical optimization, genetic evaluation and counseling, and surgical sperm retrieval in combination with assisted reproductive technologies.6,7,8,9

This special issue presents the seminal contributions of over 50 leading clinicians and scientists from 20 countries across five continents. This collection of 22 state-of-the-art reviews explores the pathophysiology, epidemiology, diagnosis, and management of NOA, offering a multidimensional analysis of current knowledge and clinical practice.

Several controversies persist when it comes to the management of NOA, and this special issue explores these issues to refine clinical approaches and optimize outcomes. It also looks into unique clinical scenarios in NOA and highlights emerging technologies that can help transform the field. Key areas of discussion include the role of varicocele repair in men with NOA, the potential benefits of hormonal optimization prior to sperm retrieval, surgical techniques and laboratory technologies to improve surgical sperm retrieval, the advantages and disadvantages of using fresh versus cryopreserved sperm, cryopreservation techniques, the feasibility of repeat sperm retrieval in cases of prior failure, clinical predictors of successful sperm retrieval, role for testicular biopsy, oncological testicular sperm extraction (onco-TESE) and ex vivo TESE, anatomical considerations in NOA, and the use of stem cells, artificial sperm, and gene editing.10,11,12,13,14,15,16,17,18,19,20,21,22

This special issue is highly recommended to clinicians involved in the care of infertile couples, including urologists, andrologists, reproductive specialists, gynecologists, embryologists, and biologists along with researchers and students in biological and medical sciences. Given the complex and multidisciplinary nature of NOA as a contributor to male infertility, many challenges remain unresolved, yet the opportunities for discovery and innovation continue to grow. Through diverse expert perspectives, the issue provides an in-depth understanding of NOA and highlights emerging directions in research and patient care. We hope readers find this special issue both insightful and inspiring as interest in the study of NOA continues to expand.
