Figure 4.

A 23-year-old female with stable keratoconus and high myopic astigmatism presented with severe visual impairment (preoperative UDVA: counting fingers at 1 m; CDVA: 1.0 with a manifest refraction of −10.00/−3.00 × 170°). At 2 years postoperatively, UDVA improved to 0.98 with a residual refraction of 0.00/−0.25 × 115°, and CDVA further improved to 1.2. (A) Preoperative corneal topography confirmed stable keratoconus, and anterior chamber depth was sufficient for phakic IOL implantation. (B) Postoperative corneal topography demonstrated maintained corneal stability. (C) Preoperative anterior segment OCT showed the baseline corneal contour with regular/irregular astigmatism and adequate anterior chamber depth. (D) Postoperative OCT confirmed preservation of the corneal surface profile and a well-centered posterior chamber ICL with an adequate vault.
