Notes:p-values are in brackets. Adjusted scores are calculated based on underlying coefficients from multivariate models that control for the same set of patient, nurse, and location characteristics. These control variables include sociodemographic characteristics of the patient (age, gender, race/ethnicity, marital status, education, expected payment source); baseline measures of patients' health and functional status, including limitations in ADLs and IADLs, physical, social, and cognitive functioning, and presence of comorbidities; baseline measures of patients' social support and living arrangements; the provider nurse's baseline characteristics, including gender, home care experience, educational level, and caseload; and environmental characteristics such as borough of residence. Standard errors have been adjusted to account for clustering effects (i.e., multiple observation on patients for a given nurse). For all outcomes, except depression, a positive difference indicates better outcomes.

Scores on the Kansas City Cardiomyopathy Questionnaire range from 0 to 100 for the summary score as well as the individual domain (subscale scores). The self-efficacy domain is not part of the summary score. For all scores, a positive difference indicates better outcomes.

Numbers shown correspond to adjusted probabilities that the corresponding underlying score was greater or equal to 50.

Numbers shown correspond to adjusted probabilities that the underlying depression score was greater or equal to six. Higher scores indicate presence of depression.

ADLs, activities of daily living; IADLs, instrumental activities of daily living.
