Failure rates were 25.5 % (13/51; 95 % CI 15.0 %–38.6 %) in the low-risk group (score $\le$ 4), 45.6 % (31/68; 95 % CI 34.0 %–57.4 %) in the intermediate-risk group ( $>\mathrm{4}$ to  $<\mathrm{8}$ ), and 87.9 % (58/66; 95 % CI 78.0 %–94.1 %) in the high-risk group ( $\ge \mathrm{8}$ ) ( $p<\mathrm{0.001}$ ). Thresholds were empirically derived in the development cohort.
