This figure shows a point-cluster map of the nine less prevalent chronic conditions (LPCCs) groups identified by the participants. Eight clusters are arranged in a circle around a central cluster, which is disease management and coordination of care. Beginning at the top of the circle, the clockwise order of the remaining eight clusters is as follows: 1) data and research; 2) partnerships and coalitions; 3) professional education; 4) community education; 5) information, referral, and support; 6) health care policy and cost; 7) planning and capacity building; and 8) assessment and evaluation.
