Clinical Takeaway

AI applications have been investigated across a broad range of perioperative pulmonary and respiratory complication phenotypes; however, the maturity and depth of evidence vary considerably across outcomes and among studies addressing the same phenotype. Although encouraging performance has been reported for several PPRCs, the evidence remains dominated by retrospective studies, with substantial heterogeneity in outcome definitions, patient populations, input data, modeling approaches, and validation strategies. Consequently, apparently similar performance across studies does not necessarily indicate comparable model validity or transferability. These recurring differences extend beyond individual phenotypes and highlight the need for cross-cutting evaluation of methodological quality, model evaluation, and clinical applicability.
