a 0 = never, 1 = sometimes, 2 = usually, 3 = always.

b 0 = not at all, 1 = once, 2 = twice, 3 = 3 times, 4 = 4 times, 5 = 5 times, 6 = 6 or more times.

c 0 = not at all, 1 = once, 2 = 2 or 3 times, 3 = about once per week, 4 = several times per week, 5 = almost every day, 6 = every day during the past month.
