Ampicillin, ceftriaxone, amikacin, erythromycin, rifampin, and sulfamethoxazole-trimethoprim were obtained from Sigma-Aldrich Chemical Co. (St. Louis, Mo.); azithromycin and doxycycline were obtained from Pfizer (Brooklyn, N.Y.); chloramphenicol was obtained from Parke-Davis (Morris Plains, N.J.); and levofloxacin was obtained from Ortho-McNeil (Spring House, Pa.).

b

Sulfamethoxazole-trimethoprim was used at a 5:1 (wt/wt) ratio.
