Core Telephone Survey Questions

1. “Why are you having this (possible)* angioplasty?”
2. “Do you think the angioplasty will improve your symptoms?”
3. “Do you think the angioplasty will help you live longer?”
4. “Do you think the angioplasty will prevent a heart attack?”
5. “What did the doctor tell you about the possible complications or problems from having an angioplasty?”
6. “When did you learn of these possible complications?” (Only asked if the patient mentioned a particular risk of the procedure)
