Proposed mechanism of bisphosphonate (BP) action. BPs inhibit a key step in the hydroxymethylglutaryl conenzyme A (HMG-CoA) reductase pathway through the inhibition of farnesyl-pyrophosphate synthase. Inhibition of protein isoprenylation results in osteoclast apoptosis and inhibition of osteoclast-mediated bone resorption.
