Assessment questions provided for each problem area as basis for care plans. Questions derived mainly from The Camberwell Assessment of Need for the Elderly (CANE)8 and EASYcare9 needs assessment tools. 2–3 page guides for each problem area. Each guide includes:
  • Background to problem area and summary of available evidence to inform service response(s)

  • Flow diagram containing guided prompts leading to definitive action points (includes cross-referencing to other reference guides and service directory).

  • Assessment scales

  • Directory of local and national services/schemes

  • Overall guidance to the assessment.
