We assessed mental and substsance use disorders as defined by the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition14 with version 3.0 of the WHO Composite International Diagnostic Interview, a fully structured, lay-administered diagnostic questionnaire used in face-to-face interviews.15 Drug abuse with dependence disorder category was omitted because of insufficient lifetime cases (n < 30) but included in the “any substance-use disorders” category.
