The final equation in a forward stepwise logistic regression analysis, with likelihood ratio test used as the criterion for determining variables to be removed from the model (n = 663).

The covariates not retained in the final equation were age, gender, change of work or work tasks between baseline and follow up that resulted in more support, and change of work or work tasks between baseline and follow up that had consequences not specifically mentioned in the checklist.

The overall rate of correct classification of the dependent variable (predicted outcome compared with the observed outcome) was estimated as 63.7%. Hosmer and Lemeshow test: χ2 = 7.603; p = 0.473.
