Organized for a dialysis patient, they are displayed chronologically under the corresponding headings. ICD-9-CM codes and descriptors, in upper case, are used to display the diagnoses and procedures. Brief comments, which had been added to the entry screens, appear in lower case beneath the ICD coded descriptors where they provide clinical color for the codes,
