(A) A glomerulus from patient 2 exhibits segmental fibrinoid necrosis, rupture of the glomerular basement membrane (arrow), and an overlying cellular crescent. There is also mild thickening of the glomerular basement membrane. (B) Ultrastructural evaluation reveals global subepithelial electron-dense deposits with focal small intervening spikes.
