Model 1 adjusted for age, gender, and race and stratified by VA site. Model 2 adds baseline PTH level, eGFR, diabetes, coronary heart disease, and the Charlson comorbidity index. Model 3 adds use of an ACEI, ARB, statin, erythropoietin, or oral calcium; body mass index; systolic BP; serum levels of albumin, calcium, and phosphate; and number of nephrology visits in the previous year.
